9/9/2023 EnchantaVaughn Approved Behavior: Client openly discussed his feelings about his current state and how he feels. Client appeared to be in a good mood as evidence by him participating in family activity without conflict. Behaviors were maintained and managed at this time. Client displays the correct manners, saying please and excuse me when he had to ask a question. Client is seen conversing and responding well to feedback. Client was observed following rules and being compliant during the session. Client remained on task and in cooperation with home rules. Intervention: QMHP implemented individualized behavior management techniques with client. QMHP observed client in his own space to determine if he required assistance. QMHP supported client displaying positive social interaction with authority. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support. QMHP coached client to acknowledge and list his strengths, resilience and the positive outcomes that often occur when he makes informed decisions opposed to impulsive ones. QMHP supported client making good decisions and having positive behaviors at home. Response: Client said he woke up feeling good and was ready to have a good day. Client expressed positive thoughts about himself. Client stated he didn't need help with anything at the moment but would say something if he needed help. Client communicated by nodding and exhibiting good eye contact. Client verbalized positive feelings about goals and objectives. Client communicated that he wants to work independently instead of as a family some days so that he could gain clarity of his personal feelings without taking it out on someone he loves. Client displayed good participation and was responsive to all instructions, interacting well with mom and QMHP.
